YTU Department of Mathematical Engineering

To ensure that all internship processes are carried out smoothly, students are required to review the 'Internship Guide' and 'Internship Flow Diagram ' below:

Document Required for Internship Start

1- FR-1877-YTÜ Staj Başvuru Formu (3 copies required)

2 -FR-1936-Staj Ücretlerine İşsizlik Fonu Katkısı Bilgi Formu

3- E-devlet üzerinden alınacak barkodlu SGK SPAS Müstehaklık belgesi

4- 2026 Calender (internship days must be clearly marked)

5- Copy of ID card

6- For internships conducted during the academic term, the student must provide their OBS course schedule

Internship start documents must be submitted to the Faculty Dean’s Office Internship Unit at least 10 (ten) days before the start date for SGK registration.

1- FR-0286-Staj Sicil Formu

2- Staj Değerlendirme Formu (Staj Bitiminde Firma Tarafından Kesinlikle Doldurulmalıdır.)

* All forms must be filled in electronically.

* No modifications should be made to the format of the forms; content must not overflow onto a second page.
